Please list anyone who may pick up the participant(s) at the end of each day of camp. If the participant(s) will be transporting themselves to and from camp, please list any adults who may pick them up if needed and be prepared for the Parent/Guardian to fill out a waiver specifying your approval for self check out at the end of the day.
